Grady's Decision Dinner Dance Event
We will be holding our first ever fundraising event in the DuBois, PA area, on April 20, 2013. The event will be a dinner-dance, with the band "The Ride" playing, at the Lifetime Celebration facility(former Lithuanian Club) located at 800 West Weber Avenue, DuBois, PA . We would love to see you attend this event as all proceeds will be benefitting Grady's Decision in supporting the families of premature babies at the DRMC NICU.
Doors open at 4pm, Dinner will be served between 5pm-7pm, with a presentation at 6pm provided by Ryan Smith. "The Ride" will play between 7pm-11pm.

Corporate Sponsorship Packages:
Gold Package: $500
Includes Name of Company on Banner that will be displayed at the Event, A Full-Page Ad in the Event Program, 8 Dinner/Dance tickets to the Event, and reserved V.I.P table.
Silver Package: $250
Includes Name of Company on Banner that will be displayed at the Event, Full-Page Ad in the Event Program, and 4 Dinner/Dance tickets to the Event.
Bronze Package + : $150
Includes Name of Company on Banner that will be displayed at the Event, and Half-Page Ad in the Event Program + 2 Dinner/Dance tickets to the Event.
Bronze Package: $100
Includes Name of Company on Banner that will be displayed at the Event, and Half-Page Ad in the Event Program.
Individual Registration: $25
Individual registration gets you a ticket to the event.

Registration is limited to the first 300 people.
Grady’s Decision is exempt under Section 501(c)3 of the Internal Revenue Code and contributions to our organization are tax deductible. The amount of the tax deduction is reduced by the value received by the donor.
