The Care Coalitions of Cuyahoga, Stark, and Summit Counties will come together for the very first time on March 1st, 2018 to address a topic that touches all of us:

SAY WHAT YOU MEAN, MEAN WHAT YOU SAY:
COMMUNICATING IN HEALTHCARE

Join other healthcare professionals from Northeast Ohio to hear from experts on patient care and communication in clinical and administrative settings. We are honored to have Dr. Eric Coleman of the Care Transitions Institute (CTI) and the University of Colorado at Denver delivering our keynote address, "An Under-recognized Key to Improving Transitional Care: Feedback Loops". We are also honored to welcome Mr. John R. Corlett, President and CEO of The Center for Community Solutions and former Director of the Ohio Department of Medicaid to our stage. In addition to presentations from experts from across the country, there will, of course, be a trademark CARE Coalition panel discussion that is worthy of such a momentous occasion!

The CARE Coalition is a movement to unite and engage healthcare professionals to more effectively and efficiently provide quality patient care, improve outcomes, and cultivate collaborative processes among the many different channels of healthcare delivery.

A portion of the proceeds from the CARE Coalition NEO Conference 2018 will go to benefit the Direction Home Akron Canton Foundation.

About the Foundation

The Direction Home Akron Canton Foundation was created with the mission of enabling older adults not eligible for government assistance to remain independent and secure at home through access to a variety of supportive services. The Foundation was developed in 1999 to help these older adults achieve their long-term care goals through the Care Coordination program.
