2019 Fall Healthy Homes Forum

The Ohio Healthy Homes Network (OHHN) 2019 Fall Healthy Homes Forum will be held on Tuesday, October 1st in the Bromfield Administration Building auditorium on the campus of the Ohio Department of Agriculture. The Forum begins at 9 am and ends at 3:30 pm. Registration opens at 8 am.

   Keynote Speaker

Amy Acton, MD, MPH, was nominated director of health for the Ohio Department of Health (ODH) by Governor Mike DeWine in Februrary 2019. A licensed physician in preventative medicine with a master's degree in public health, Dr. Acton has more than 30 years of experience in medical practice, government and community service, healthcare policy and advocacy, academic and nonprofit administration, consulting, teaching, and data analysis.

 
Luncheon Speaker
  Councilman Blaine A. Griffin represents Ward 6, one of
  Cleveland's most diverse wards, encompassing several
  East Side neighborhoods. As chairman of council's Health
  & Human Services Committee, Councilman Griffin
  spearheaded passage of Cleveland’s history-making lead
  poisoning prevention legislation on July 24, 2019. Prior to
  serving on council, Councilman Griffin had been
  executive director of the city's Community Relation's Board 

  for 11 years.

Information on Registration

Location and Time

The Fall Forum will take place from 9 am to 3:30 pm in the Bromfield Administration Building on the campus of the Ohio Department of Agriculture.

Meeting space at ODA is limited. Registration will end at 110 participants.

Registration Fees

Registration begins at 8 am outside the auditorium in the Bromfield Administration Building. Please bring a copy of your registration receipt with you to assist us with registration.

Early bird registration fees are $50 for OHHN members and $80 for non-members. Full-time students are $35. Rates increase by $10 on registrations received after September 15th. All fees must be paid at registration unless arrangements have been made in advance.

On-Site Registration: Anyone who registers at the door on October 1st and hasn't made prior arrangements with OHHN will be charged an on-site registration fee of $20 in addition to the member or non-member fee.

Payment through Pay Pal is the preferred method of payment. 

If you choose to pay by check, please note that OHHN must receive either a Purchase Order or a check prior to the Forum. Contact Patricia Barnes, OHHN Executive Director if you need an invoice and/or to make special payment arrangements (ohhn@ohhn.org).

OHHN Members Receive Discounted Rates

Is your agency an OHHN member?  If not, join now and get the discounted rate for members. Use OHHN’s Membership Application and pay by check, or join on the OHHN website. Note that OHHN Agency members can register an unlimited number of participants at the discounted rate. Contact OHHN for more information.

Cancellation Policy

Refunds will be honored if OHHN receives the cancellation before September 20th. No refunds will be available after September 20.

Participants who register, do not attend and fail to cancel their registration by September 20th will be invoiced for the registration fee.

September 27 - On-line registration ends  (registrations after September 27 must be sent to OHHN prior to October 1 and will be based on space availability).

Lunch

A lunch buffet will be served from 11:45 am to 12:30 pm and is included in your registration fee. Please indicate on the registration form if you plan to participate and have any special dietary needs.

Scholarships

Partial and full scholarships are available. To request a scholarship, write to OHHN.

Parking

Parking is free at the Ohio Department of Agriculture. Please park in the sections marked for Visitors.
