The Utah State Survey Agency, Health Services Advisory Group (HSAG) End Stage Renal Disease (ESRD) Network 15, the University of Utah, Fresenius Kidney Care, and Intermountain Health invite you to join them for this unique educational opportunity provided at no charge.

Topics to be addressed during this informative, one-day session, and some of their objectives, include:

• State Survey Readiness

–     Defining the purpose and approach of the ESRD Core Survey process.
–     Identifying the goals of the ESRD Core Survey.
–     Describing previous survey and deficiency data for dialysis facilities in Utah

• Emergency Preparedness

–     Demonstrating knowledge of the EP Final Rule, and its Four Core Elements
–     Explaining when 1135 Waivers can be activated and how to request a waiver or flexibility.
–     Demonstrating how E-tags have been cited and how surveyors determine compliance.

• Home Modality Education
• Infection Prevention in ESRD Facilities
• Transplant Awareness
• Patient and Family Engagement

Up to Five Continuing Education Credits Are Available
