Evaluation – Please note that the evaluation will be forwarded to members by email following the presentation. It will be sent to the email address you used to register. It will ONLY be sent to the email used at the time of registration. CE certificates for CCM and SW will be available for immediate download following the completion of the online survey. RN* Certificates may be sent via separate email within 2 weeks of the close of the online survey.

All members and potential members are welcome; however, CE’s are restricted to members only. All memberships previously established now require renewal. Renewal for 2018 is $50 for the year. Memberships may be renewed online via credit card, by mail via check or in person via cash, check or credit card. Membership applications will be available on site. Dinner is being provided compliments of Mary Free Bed.

Please note that potential sponsors may attend a single meeting to acquaint themselves with the group and determine the potential for sponsorship. Potential sponsors will be charged a modest fee of $30 to cover the cost of the dinner. All potential sponsors must register and pay the fee online.

Please note the new cancellation process. In the event that you will not be able to attend, you MUST notify us of your cancellation. This can be done by email to tory@cacmc.org, or by phone or text message at 517-803-2261. We must be notified of your need to cancel prior to 4/12/18 at noon. No-shows or late cancellations may incur a $30 fee.

Desired Outcome:

Case Management Professionals will acquire knowledge of the ongoing complex care needs of patients with prosthetic devices from the early post-operative time going forward to develop an individualized plan of care that supports maximum functioning with available resources. 

Learner Objectives: Upon completion of this educational offering, participants will be able to:

  1. Describe factors that influence the choice of devices that are selected for an individual patient
  2. Explain the sequence of care that maximizes the patients functional outcome when a prosthetic device is to be utilized
  3. Describe the new technologies and devices available to patients who require adaptive prosthetics for knees, feet and hands.  

This course is designed for: Case Managers, Nurses, Social Workers, Vocational Counselors, Psychologists, Physicians, Medical and Rehabilitation Providers

Cost: Free but CEs are available only to members. Membership is $50.00 a year and applications are available at the meeting or by contacting Susan Wisser, RN, MSN, MBA, CCM at (517) 364-3936.

Continuing Education: 1.25  Contact Hour of Continuing Education is offered for Registered Nurses, and 1.00 Continuing Education Contact Hour for Certified Case Managers or/and Social Workers.

Michigan State University College of Nursing (OH-294, 08/01/18) is an approved provider of continuing
nursing education by the Ohio Nurses Association (OBN-001- 91), an accredited approver by the American Nurses Credentialing Center’s Commission on Accreditation.

To receive CNE or CE Certificate of Completion, participants must sign in prior to the presentation and only social workers will need to sign out after the presentation. Social workers must provide their license numbers when signing in. The Certificate of Completion will be provided to participants after they complete an on-line survey post-meeting.

Approved Provider status does not imply endorsement by the provider, ANCC, OBN or ONA of any products displayed in conjunction with an activity.

No Conflict of Interest Statement: CACMC board members, including those members responsible for conference planning, their spouses and/or partners, have not controlled or influenced the content of the educational activity and have no financial relationship with a commercial interest, the products or services of which are pertinent to the content of the educational activity.

 Grievance Procedure: Disputes of grievances over the program or its policies or practices can be directed to the Education Chairperson, Jill Clouser, LBSW by contacting jill@cacmc.org. All efforts will be made to solve the dispute.

New Process for CE distribution

  1. All registrations must be completed online
  2. All attendees must sign in for the presentation. Social workers must sign in and out.
  3. Within 10 (ten) business days following the presentation, you will receive an email from CACMC at the address used to register. The email will contain a link to the program evaluation survey
  4. You MUST complete the program evaluation survey online in its entirety and click “submit”. If you are collecting more than one type of continuing education, you will be required to complete a program evaluation survey for each credit you are requesting
  5.  At the completion of the program evaluation survey, CCM and SW will have the opportunity to print their certificates online. RN’s will receive their certificates via a separate email. This email will come from MSU directly, approximately 2 weeks after the survey closes.
