10:00 to 11:30 - Consideration of Palliative and Hospice Care in Patient
with Addiction Concerns

Summary of Presentation:
Aging does not deter or nor eliminate drug or alcohol abuse, and late onset substance abuse is primarily related to medical problems. Prescription drug misuse and abuse is a growing problem among older adults, and nearly 25% of hospice patients have a substance use disorder. Additionally, there is concern growing around the availability of opioids in hospice care as diversion of medication by family and caregivers can potentially be a problem. This course aims to educate on the risks of addiction in the elderly, identifying risk factors for patients with a history of addiction and identifying interventions to reduce the likelihood of medication diversion.

Learning Objectives:
1. Learner will be able to identify risks of addiction in the elderly population.
2. Learner will understand risk factors for patients with history of addiction on hospice and palliative care.
3. Learner will be able to describe interventions to curb abuse and reduce the likelihood of medication diversion.

Presenter: Christy York, RN, MSN 

Christy York is the Regional Nursing Coordinator for the southeast region at Transitions Hospice. She has been a registered nurse for more than 11 years. Most of her career was spent in the skilled nursing setting, both in clinical and administrative roles. Her love of learning and teaching others brought her back to Chamberlain University College of Nursing to earn her Master’s in Nursing Education. She was always drawn to hospice nursing as she worked closely with many hospice teams in the skilled nursing setting. She noticed the difference it made for patients and their loved ones when they had a hospice team during one of the most special and fragile times in their lives. When the opportunity arose to become part of the Transitions family, Christy was honored and gladly accepted. Her role at Transitions Hospice allows her to educate not only the staff she manages daily, but patients, families, and facility staff, along with members in the surrounding communities.

11:45 am to 1:15 pm - Consideration of Palliative and Hospice Care in Patient with Addiction Concerns

Summary of Presentation:
Grief is a multifaceted response to loss, particularly to the loss of someone or something that has died, to which a bond or affection was formed. Although conventionally focused on the emotional response to loss, it also has physical, cognitive, behavioral, social, and philosophical dimensions. Complicated Grief exists when normal grief reactions become so severe and last so long as to significantly impair one’s functioning in the world: socially, occupationally, and in activities of daily living. This presentation aims to help the clinician identify risk indicators for complicated grief, become familiar with assessment tools that can be used to determine increased risk and learn interventions and strategies to improve bereavement care planning.

Presenter: Katie Monahan

With 15+ years in the healthcare industry, Katie Monahan is a graduate of Alverno College where she earned her Bachelors of Music in Music Therapy. Shortly after college she left the field and began her career as a Resident Social Service Coordinator for dual diagnosis seniors; worked as a property manager in low-income senior housing; was the Marketing Director in skilled home health care; and assisted in opening an inpatient psychiatric hospital in northern Illinois. A commitment to respect and servant-leadership as organizational values brought her to her current role with Transitions Hospice in 2019. As the Manager of Business Development for Transitions Hospice Katie provides continuing healthcare education for both the professional and consumer, builds partner relationships and guides prospective patients through the palliative care and hospice process. As a believer in the importance of volunteerism, Katie serves in several volunteer leadership positions including President of Continuity of Care – Lake McHenry Chapter; President of the Senior Service Coalition of Lake County; Vice President of the Lake County Behavioral Health Alliance; founding member of the Coalition for Aging Resources & Education; and as the past Corporate Sponsorship Chair for the Lake County Walk to End Alzheimer’s. She has also served on several professional advisory committees for both hospice and not-for-profit home health agencies and on the Women’s Board at First Presbyterian Church of Libertyville. When not working, Katie enjoys travelling both locally and abroad in search of the perfect oyster, learning about craft distilleries and cheering on the Chicago Cubs.
