Care Manager - Full Time

Jewish Family Services is offering an opportunity for TWO full-time Care Managers. These positions will be responsible for providing assessments, case management services, advocacy and ongoing monitoring of services to elderly or disabled persons enrolled in JFS’s My Choice Family Care program.

Qualified candidates must have a four-year bachelor's degree in a social services area (e.g. social work, rehabilitation, psychology, etc.) as well as at least one (1) year of paid formal (non-family setting) experience working with specific populations (e.g. Developmentally Disabled adults, Physically Disabled adults, Geriatric) in social service care management, criminal justice, or related social service experience, or have a four-year bachelor's degree in any other area with a minimum of three (3) years of paid formal (non-family setting) experience working with developmentally disabled adults, physically disabled adults, or geriatric individuals. Candidates who have prior experience working with individuals with mental health issues are preferred.

Candidates must have a working knowledge of standard Microsoft Office applications and the ability to perform Internet searches. Familiarity with social work methodology is preferred. Strong organizational and interpersonal skills required. Candidates must have the ability to respect and maintain privacy and dignity of members and assure member confidentiality at all times. Ability to collaborate and work effectively with clients, volunteers, agency staff and others is required. Knowledge, understanding and/or willingness to learn Jewish culture and traditions are necessary. A valid driver's license AND access to a car with insurance is required. Successful completion of a criminal background check with the Wisconsin Bureau of Justice, Crime Information Bureau is also required.

 

Director of Social Services - Full-time

Jewish Family Services is offering an opportunity for a full time Director of Social Services to provide oversight of the day-to-day administrative and operational activities of the Social Services department consistent with contractual requirements, legal and ethical best practices and agency policies. This position oversees all department activities, programs, services and staff, as well as ensuring clients receive quality, cost effective services.

As a member of the Senior Leadership Team, the Director supports and enhances organizational, leadership and administrative performance of the team and agency, and participates in organizational activities (grant writing, development activities, strategic planning, etc.) as part of this role.

Qualifications:

At a minimum, the Director of Social Services shall:

  • Possess a Master’s Degree in Social Work or a related Human Services discipline combined with 5 years of case management experience and 3 years of supervisory experience in a case management/community services setting.
  • Possess a working knowledge of various Case Management systems (TCM, CSP, Wraparound Milwaukee, My Choice Family Care, Comprehensive Community Services, etc.).
  • Exhibit demonstrated leadership ability and display and maintain high standard of professional conduct for self and staff.
  • Possess excellent organizational, critical thinking, problem solving, creative and independent thinking.  
  • Exhibit demonstrated cultural competency/humility and a commitment to diversity management.
  • Have familiarity with Electronic Health Record (EHR) management systems.
  • Be proficient in computer use, Microsoft Office applications, service provider documentation systems, EMR systems and internet searches.
  • Possess knowledge, understanding, and/or willingness to learn Jewish culture and traditions, as well as those of persons served.
  • Be able to respect and maintain client privacy and dignity and assure client confidentiality at all times.
  • Be able to collaborate and work effectively with clients, volunteers, agency staff and others in a team concept.
  • Possess a valid driver's license AND access to a car with insurance.
  • Have the ability to successfully complete a criminal background check with the Wisconsin Bureau of Justice, and a caregiver background check with the Wisconsin Department of Health Services, WI Department of Children and Families, and WI Department of Safety and Professional Services.
