Registration: Pre-registration online is required by December 4, 2020 (link below). All registrants will be provided with a Zoom link and instructions to attend the live webinar the day before and day of the event. Please make sure to register with an email you can access on the day of the webinar.
Cost of Attendance: The cost of attendance is $75.00 for MD/DO and $60.00 for non-MD/DO. Early bird discounts in October. Use code EARLY25 to receive 25% off registration fee before November 1, 2020. Secure payment is available online using Visa, MasterCard or American Express. No same day registration is available; advance registration is required. Refunds are available up until one day prior to the event (minus a $5.00 processing fee). No refunds available on or after December 4, 2020.
Accreditation: This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint providership of OHSU School of Medicine and the Slocum Research & Education Foundation. The OHSU School of Medicine is accredited by the ACCME to provide continuing medical education for physicians.
Credit: OHSU School of Medicine designates this live activity for a maximum of 4 AMA PRA Category 1 CreditsTM. Physicians should claim only the credit commensurate with the extent of their participation in the activity.
BOC Credit: Donald B. Slocum Research & Education Foundation (BOC AP# P3302) is approved by the Board of Certification, Inc. to provide continuing education to Athletic Trainers. This program is eligible for a maximum of 4 EBP Category hours/CEUs. ATs should claim only those hours actually spent in the educational program.
