PeaceJam Miami Slam Individual Registration

This form is for individuals coming the to the PeaceJam Miami Slam.  In addition to this online form, we will need a paper form for each participant that is signed by their parent and/or guardian.  Please email or bring a physical copy of these forms for each participant.  Youth that do not have a signed form by their parent/guardian, will not be able to participate!!

The registration fee for the conference is $20 per person.  Please note that, if needed, we can provide financial support upon request.

Due to space limitations, we are asking that you submit payment within 10 days after registering. If we do not receive payment within 10 days of your registration you will be added to a waiting list. Priority will be given to those whose payments are received first.
