The American Association of Diabetes Educators (AADE)
is the accredited provider for continuing education.

This activity is jointly provided by the American Association of Diabetes Educators (AADE)
Kentucky Coordinating Body (CB) and Local Networking Groups (LNGs)
(GLADE, KADE, TRADE, DECA), the Kentucky Diabetes Prevention and Control Program (KDPCP),
and the Kentucky Diabetes Network (KDN).

TARGET AUDIENCE AND ACCREDITATION INFORMATION

This symposium is a knowledge-based offering designed for nurses, dietitians, pharmacists, certified diabetes educators, and other healthcare professionals involved in the education and care of people with diabetes.

Requirements:
To successfully complete the program and receive credit, participants must pay the required fee, sign the attendance roster, attend the entire program, and complete the evaluation form.

Accreditation Statement:
The American Association of Diabetes Educators (AADE) is the accredited provider for continuing education as indicated below. AADE approval of programs or materials does not constitute endorsement of a provider, program, or material. In addition, approval of this educational offering by AADE does not imply endorsement of specific therapies, treatments, or products discussed in the presentations. 

Nurses:
AADE is accredited as a provider of continuing nursing education by the American Nurses Credentialing Center’s Commission on Accreditation. This program provides 6.0 contact hours of continuing education credit. AADE is also approved by the California Board of Registered Nursing, Provider Number 10977.  This program provides 6.0 contact hours. 

Pharmacists / Pharmacy Technicians:
AADE is accredited by the Accreditation Council for Pharmacy Education (ACPE) as a provider of continuing pharmacy education. This program provides 6.0 contact hours (.60 CEUs) of continuing education credit. ACPE Universal Activity Number: 0069-9999-19-298-L01-P Effective Date: ;November 15, 2019 to November 15, 2020.

Dietitians:
AADE (AM001) is a Continuing Professional Education (CPE) Accredited Provider with the Commission on Dietetic Registration (CDR). CDR Credentialed Practitioners will receive 6.0 Continuing Professional Education Units (CPEUs) for completion of this activity / materials. Continuing Professional Education Provider Accreditation does not constitute endorsement by CDR of a provider, program, or materials.

Certified Diabetes Educators:
To satisfy the requirements for renewal of certification for the National Certification Board for Diabetes Educators (NCBDE), continuing education activities must be diabetes related and approved by a provider on the NCBDE list of Approved Providers (www.ncbde.org). NCBDE does not approve continuing education. The American Association of Diabetes Educators (AADE) is on the NCBDE list of Approved Providers.

Continuing Education (CE) Statements of Credit will be available at the end of the conference for those who meet the requirements.
Planners / Content Reviewers disclose no conflict of interest relative to this educational activity. Presenter disclosures will be announced during the educational activity.

LEARNING OUTCOME / OBJECTIVES
Symposium attendees will have increased knowledge of the specified content areas presented and will, therefore, choose at least one concept, practice area, technique, or tool that they will utilize to enhance the care and or education for individuals with or at risk for diabetes.
