The Association of Diabetes Care & Education Specialists (ADCES)
is the accredited provider for continuing education.

TARGET AUDIENCE AND ACCREDITATION INFORMATION

This symposium is a knowledge-based offering designed for nurses, dietitians, pharmacists, certified diabetes educators, and other healthcare professionals involved in the education and care of people with diabetes.

Requirements:

*To successfully complete the program and receive credit, participants must pay the required fee, sign in to each session through the Whova App, attend the entire program, and complete the online evaluation form.

Accreditation Statement:

In support of improving patient care, this activity has been planned by Kentucky Diabetes Network and the Association of Diabetes Care & Education Specialists. Association of Diabetes Care & Education Specialists is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC) to provide continuing education for the healthcare team.

This activity was planned by and for the healthcare team, and learners will receive 6.0 Interprofessional Continuing Education (IPCE) credits for learning and change.

American Nurses Credentialing Center (ANCC)

Association of Diabetes Care & Education Specialists designates this activity for a maximum of 6.0 ANCC contact hours.

The Association of Diabetes Care & Education Specialists is approved by the California Board of Registered Nursing, Provider Number 10977, for 6.0 contact hours. RNs must retain this document for 4 years after the activity concludes.

Commission on Dietetic Registration (CDR): CDR Credentialed Practitioners will receive 6.0 Continuing Professional Education units (CPEUs) for completion of this activity. Completion of this RD/DTR profession-specific or IPCE activity awards CPEUs (One IPCE credit = One CPEU). If the activity is dietetics-related but not targeted to RDs or DTRs, CPEUs may be claimed which are commensurate with participation in contact hours (One 60 minute hour = 1 CPEU. RDs and DTRs are to select activity type 102 in their Activity Log. Performance Indicator selection is at the learner’s discretion.

Certified Diabetes Care and Education Specialists: To satisfy the requirements for renewal of certification for the Certification Board for Diabetes Care and Education (CBDCE), continuing education activities must be diabetes related and approved by a provider on the CBDCE list of Approved Providers (www.ncbde.org). CBDCE does not approve continuing education. The Association of Diabetes Care & Education Specialists is on the CBDCE list of Approved Providers.

Continuing Education (CE) Statements of Credit will be available at the end of the conference for those who meet the requirements.
Planners / Content Reviewers disclose no conflict of interest relative to this educational activity. Presenter disclosures will be announced during the educational activity.

LEARNING OUTCOME / OBJECTIVES

This symposium will enable participants to:

Symposium attendees will attain knowledge of social determinants of health, changing clinical guidelines and healthcare delivery methods in the changing environment of diabetes management.

 This symposium will enable participants to:

  1. Define social determinants of health.
  2. List at least 2 ways social determinants of health play a role in self-management.
  3. Explain how to customize approaches to meet clients where they are.
  4. Describe two benefits for building/participating in a quality improvement coalition from KY Diabetes Learning Collaborative lessons learned.
  5. Identify one quality improvement methodology that can be utilized to improve patient outcomes.
  6. Discuss two ideas to help improve outreach to and coordination with providers/practice/patients about the importance of DSMES.
  7. Summarize first-hand and lived experiences related to DSMES from a primary care practice, Diabetes Self-Management and Support (DSMES) provider and patient(s).
  8. Describe the health inequities faced by adults with disabilities across the state.
  9. Identify gaps in accessibility within diabetes prevention and education programs.
  10. Discuss strategies and resources which can increase accessibility and inclusion of existing diabetes programs.
  11. Describe the relationship between diabetes, chronic kidney disease (CKD), and heart failure.
  12. Discuss medication use for individuals with diabetes and CKD or heart failure.
  13. Develop an evidence-based treatment plan for an individual with diabetes and CKD or heart failure.
  14. Describe the state of food insecurity and food scarcity and the impact on people with diabetes.
  15. Assess people with diabetes for signs of food insecurity and identify those a high risk.
  16. Identify resources for food insecure populations in your community.
  17. Design treatment plans that align with each client’s needs and connect them to community resources.
  18. Discuss the epidemiology of covid-19 and outcomes related to chronic metabolic diseases of interest.
  19. Discuss the fascinating pathophysiology of COVID-19 and diabetes as it relates to pathophysiology and therapeutics.
  20. Identify strategies to improve successful implementation of successful telemedicine in diabetes care.
