Join HealthInsight's  Quality Improvement Program, IC³ Beacon Community, and Regional Extension Center for our first Learning and Action Network series. 


We invite you and your colleagues to this landmark event in Provo where we will jump-start our community collaboration to promote the best in patient-centered care.


We will start the day with a morning plenary followed by learning and action networks.

    • How to implement office process changes that promote population based care,implement best practices, engage patients in their own care, improve hypertension screening, lipid screening, and blood pressure control.
    • How to implement patient coaching practices that prevent hospital readmissions and improve successes in transitions of care.
