Designing a whole church approach to safety for your unique church setting led by Judy Wasburn and HomeGrown Faith: create a seamless journey for faith formation that connects each generation, age and stage of life led by Linda Staats. Saturday & Sunday - great workshops, worship, rest & renewal! Health Form & Waiver

Please complete this HEALTH FORM and WAIVER. A completed health form is required for each participant to attend.This form is kept only for medical emergencies and therefore we encourage you to include with it a list of all current medications and a copy of your health insurance card(s) (front and back). This form can be kept in a sealed envelope, if you desire, and returned to you at the end of the retreat. The waiver however, must be turned in at the beginning of the event.
