December 3rd, 10am-1pm

Purpose of Event

A win-win-win event...

• Vendors/Providers have the opportunity to network with Hospital Case Managers, Social Workers and Discharge Planning support staff.

• Hospital staff are able to gather resource information, while obtaining Continuing Education Credits.

• Proceeds from the booth fees will be used to support low income patients in need.

Resource Fair Agenda

10 - 10:30am Set-up, Vendors arrive at 10 

10:30am State of the Union, by Case Management Staff

11am - 1pm Event, Vendor Fair (lunch provided for hospital staff)

Registration Options:

When registering please select one of the following registration options.

• CEU poster presentors & Marketing (1/2 table) $150
  *if you print your own poster, you may deduct -$75
  
• Marketing Materials only (1/2 table) $150
  
• Community Resource table (shared table with brochures only) $25
