Lil' Pals T-ball Academy - Spring 2012 

The Lil’ Pals T-ball Academy is a 6-week instructional sports academy for 3-6 year olds. Our parent volunteer coaches will teach your child the fundamentals and basics of the sport while promoting teamwork, safety and most of all, fun! This is a recreational sports program.

Registration Fee: $30 per child and includes a program t-shirt, cap, team picture and trophy
Register up to 4 children from the same family on this form.

By registering my child(ren) for this program, I am permitting my child to participate in the Paladin Sports Outreach Lil' Pals T-ball Academy.

I recognize there are risks associated with strenuous physical exertion when engaging in a sports program, which may result in serious injury or death. I certify to the best of my knowledge, my child's physical condition is satisfactory to participate in physically demanding activities and if in doubt of my child's condition I have been advised to seek the advise of a competent physician and abide by his/her advice. In case of injury, I authorize the Paladin Sports Outreach instructor to summon professional help for treatment or transportation.

I, the undersigned, have read this release and assumption of risk and understand all its terms. I execute it voluntarily and with full knowledge of its significance. I hereby represent that I am at least 18 years of age and legally capable of executing this release and assumption of risk.

WHEN ENTERING YOUR CHILD'S INFORMATION, PLEASE DOUBLE-CHECK ALL FIELDS FOR ACCURACY, ESPECIALLY THE DATE OF BIRTH. IF YOUR CHILD'S DOB IS ENTERED INCORRECTLY AND THEY ARE PLACED ON A TEAM IN THE WRONG AGE GROUP, YOU MAY FORFEIT THEIR PLACE IN THE PROGRAM IN WHICH CASE YOUR REGISTRATION FEE WILL BE REFUNDED TO YOU.
