NCQA PCMH 2017 Standards Training

Join us for NCQA PCMH 2017 Standards Training June 12-13, 2018 in Helena, MT. The 1.5 day training will cover the 2017 standards, redesigned recognition process, annual reporting requirements and the Behavioral Health Distinction requirements. Susan Crocetti, NCQA PCMH Certified Content Expert from Qualis Health, will be the featured presenter. The training will also include Office Hours to answer questions from individual health centers.

Nursing Continuing Education:
  • This continuing nursing education activity was approved by Montana Nurses Association, an accredited approver with distinction by the American Nurses Credentialing Center’s Commission on Accreditation.
  • Approved for 8.5 contact hours

Behavioral Health Continuing Education: Pending

  • Interpret the intent of the NCQA 2017 PCMH standards;
  • Discuss the transformational concepts the standards support;
  • Review the newly re-designed survey process and submission requirements;
  • Receive practical advice on how to operationalize the changes needed for NCQA recognition;
  • Learn methods and tools to manage the project work associated with the recognition and survey submission process.

Registration Fees:
Registration cost is waived for CHC staff. Non-CHC staff will be billed $350 by MPCA. The bill will be sent to the email address of registrant.

Susan Crocetti, RN, BSN, CPHQ, CPEHR, HEM, PCMH CCE Qualis Health, Senior Consultant Susan Crocetti provides coaching and technical assistance to practices seeking to adopt a more patient-centered approach to care delivery and improve overall clinical operations. She also helps them develop practice transformation readiness tools and prepare for NCQA or Joint Commission PCMH surveys. Ms. Crocetti has over 20 years of industry experience in areas such as healthcare quality improvement, health information technology, patient safety, risk management, staff education, regulatory compliance and clinical care. She is an NCQA PCMH Certified Content Expert. Her many areas of expertise include team development, care management for complex patients, quality improvement, regulatory compliance, patient safety, Joint Commission, practice transformation and NCQA PCMH standards.
