Christmas Basket Sponsor Form 

Thank you for your interest in sponsoring a family! Typically a sponsor will provide the items on a list of recommended grocery items as well as one gift item for each child in the family being sponsored. If you would prefer to only provide the food items, or only provide gift items, please clearly type your preference on the form.  The additional information requested on the form is designed to help us match you with a family in need. We cannot guarantee a perfect match of all your selected preferences, but it will help us to do our best. Christmas baskets will be distributed to families Friday, December 8.

Please fill out the sponsor registration form by 5:00 p.m. December 7, 2017.                                             

We will provide the information of the family with whom you have been matched.

We have lists for suggested food items: families will provide a list of suggested gift items.

To keep information confidential, we will provide you with a number identifying the family you are sponsoring.

Wrap the gifts if you wish or provide wrapping paper, tape and tags for the family. Please have all items clearly marked with the family’s number.  (If you wrap the gifts, be sure to indicate whom it is for, e.g. girl age 5.   Gift receipts can be helpful also if color or size is a question.)

Bring food/gifts December 8 between 7:00 a.m. and 12 noon to Door 8 at St. Stephen’s.
