Heart Failure and Cardiac Rehab

Trilogy Home Healthcare 

This virtual one-hour continuing education credit will educate nurses about heart failure pathology, medical treatments, and cardiac rehab stages and discuss the bridge of home health services from transition between inpatient and outpatient.

Learner Objectives: 

  1. Describe the pathophysiology, symptoms and diagnostic procedures of coronary artery disease in heart failure.
  2. Demonstrate an understanding of the basic treatments of heart failure.
  3. Describe the various phases of cardiopulmonary rehabilitation.
  4. Identify the benefits of cardiac rehab.
  5. Appreciate the bridging benefits of home health services for the heart failure patient.
  6. Guide and educate patients living with heart failure to maximize their independence.
