Community Collaborative

Our next meeting for the Albuquerque Metro Transitions of Care Community Collaborative will be on Friday, August 7 from 1:00 to 4:30 p.m. at the New Mexico Hospital Association (NMHA) at 7471 Pan American Fwy in Albuquerque.

 Intended audience:

• Hospital leadership (case/care management staff, quality teams)
• SNF leadership (medical directors, administrators, directors of nursing)
• Primary care physician offices, home health agencies, dialysis centers and other community partners 

Goal and meeting objectives:

Our goal is to continue building a community coalition engaged in improving the coordination of care for Albuquerque Metro residents across settings.

At this meeting, the coalition will:

1. Assess the gaps and barriers in transitions of care between Navajo Area IHS hospitals and Tertiary Care Centers in Albuquerque

2. Establish a charter and next steps based on this assessment to align and support efforts of coalition members

3. Identify and align ongoing collaborative efforts to firmly address improving communication between and among healthcare organizations, providers, and patient/family with a focus on identifying processes that address continuity of care
