Breastfeeding Medicine: From Evidence to Action


Please join us for a day-long seminar focused on supporting breastfeeding for term, preterm, and NICU babies and their parents. 

Dr. Wight will discuss breastfeeding support, hospital procedures and policies in normal and challenging situations and how to plan and support breastfeeding after discharge. 
Nancy E. Wight MD, IBCLC, FABM, FAAP
Nancy is an attending Neonatologist with San Diego Neonatology, Inc. and is theMedical Director for Sharp HealthCare Lactation Services in San Diego, CA. Sheis board-certified in Pediatrics (Univ. of North Carolina, Chapel Hill) andNeonatal-Perinatal Medicine (Univ. of California San Diego) and has been anIBCLC since 1988. She is currently secretary and education coordinator for theSan Diego County Breastfeeding Coalition, which she helped found. Nancy isthe Breastfeeding Coordinator for District IX, Chapter 3 of the AmericanAcademy of Pediatrics. She is a past president of the Academy of BreastfeedingMedicine, and on Board of HMBANA. She reviews research articles for severalprofessional journals. Her book, “Best Medicine: Human Milk in the NICU” waspublished in July 2008. She was awarded a 2014 Golden Wave Award by theCalifornia Breastfeeding Coalition for her efforts to reduce obstacles tobreastfeeding in California and the WIC Breastfeeding Champion Award in 2017.She retired from full-time employment January 2018.

1. Managing Breastfeeding in the NICU

Session Description:

 Knowing that human milk is the best medicine for NICU infants (and all infants) is the first step, but achieving human milk feeding and direct breastfeeding can be difficult.  We will discuss evidence-based steps to establish and maintain a mother’s milk supply and transition an infant to breastfeeding.  Consistency and teamwork are key issues in establishing successful breastfeeding in the NICU.

Objectives:

 At the conclusion of the presentation the participant will be able to:

  • Discuss methods of establishing & maintaining mothers’ milk supply.
  • Identify research-based strategies to facilitate successful breastfeeding for preterm mother/infant dyads.
  • List at least 3 barriers to breastfeeding for NICU infants.
  • Discuss NICU policies & procedures which can support or undermine breastfeeding.
  • Recognize the key principles of the Neo-BFHI.

2. NICU Graduate Nutrition and Breastfeeding

Session Description:

The VLBW infant at the time of discharge is a nutritional challenge to healthcare providers in terms of what type of milk should be given and whether human milk will need fortification or supplementation.  Post-discharge recommendations and practices vary widely by Country and region of the US. Preterm infants tend to be discharged before reaching term corrected age and before breastfeeding is well established.   The goal in nourishing preterm infants after discharge is to promote human milk feeding, minimize nutrient deficits, promptly address any nutritional deficits, and avoid over-nourishing

Objectives:

At the conclusion of the presentation the participant will be able to:

  • Summarize benefits and risks of human milk for preterm infants post-discharge.
  • List monitors of nutritional adequacy for preterm infants.
  • Describe at least 3 components of nutritional discharge planning for NICU graduates.
  • Discuss possible methods of facilitating transition to full breastfeeding post-discharge

3. Free Hookup with Every Delivery: Evidence-Based Hospital Breastfeeding Care

Session Description:

 Hospital breastfeeding support is a key component of establishing a good milk supply and continued and exclusive breastfeeding as recommended by the AAP, ACOG, AAFP, CDC, WHO/UNICEF and virtually every organization involved in perinatal issues. Physicians and other healthcare providers' recognition of breastfeeding basics and the evidence behind the BFHI Ten Steps can make a vital difference in breastfeeding success. 

Objectives:

At the conclusion of this presentation the participant will be able to:

  • Briefly explain the normal physiology of milk production.
  • Describe the normal newborn breastfeeding course in regards to feeding behavior, weight loss and gain, and stool progression.
  • Recognize how some current hospital policies and practices may interfere with optimal breastfeeding.
  • List at least 3 ways the primary care provider can promote and support breastfeeding.
  • Outline resources physicians can use to support breastfeeding in the hospital and the community.
4. Managing Infant Concerns: Hypoglycemia, Jaundice & More

Session Description:

There is a difference between recommending a mother breastfeed and actively promoting and supporting breastfeeding, especially when the mother is experiencing difficulties.  We will explore breastfeeding-supportive ways of addressing common  infant challenges to establishing and maintaining breastfeeding.
Objectives:

At the conclusion of the session the participant will be able to:

1.  Describe the normal newborn breastfeeding course in regards to feeding behavior, weight loss and gain, and stool progression
2. Describe a breastfeeding supportive approach for treatment of hypoglycemia
3.  Discuss breastfeeding-supportive management of newborn Hyperbilirubinemia.
4.  Identify at least 3 maternal and 3 infant causes for slow weight gain or FTT while breastfed
5.  Discuss how unnecessary supplementary feedings may interfere with establishing breastfeeding

Written cancellations received one week prior to the event will be granted subject to a $5 fee. No refunds after this date.

Continuing Education Credits (offered at no additional charge)

Registered Nurses: BreastfeedLA is an approved provider by the California Board of Registered Nursing. This course offers 6 contact hours of continuing education.  BreastfeedLA’s provider number is CEP 16435. Licensee must maintain certificate for a period of four years.

Lactation Consultants: BreastfeedLA is an approved Long-Term Provider by the International Board of Lactation Consultant Examiners #CLT113-36. 5 L-CERPs will be awarded.
