This summit is provided by the Northern and Southern New Hampshire Area Health Education Centers in partnership with the New Hampshire Community Health Worker Coalition.

The New Hampshire Community Health Worker Coalition Aims to: 
  • Promote community health workers as vital members of the health care and social service team
  • Advocate and educate
  • Leverage opportunities to educate stakeholders about CHW model
  • Lead and advocate for policy change

 

Community Health Workers are...
  • Lay people who are members of a community and have a vast understanding of the community they serve in;
  • Facilitate help with health, social services, and can access services to improve the quality and cultural competence of those they encounter; and
  • Build individual and community capacity by increasing health knowledge and self-sufficiency in various ways such as outreach, community education, informal counseling, and social support and advocacy.

At the end of the full-day conference, participants will be able to:

  • Describe CHW initiatives in New Hampshire.
  • Explore the leadership role of CHWs in the community.
  • Frame messages to build public awareness of CHWs.
  • Identify ways to measure social determinants of health for value-based payment models.
  • Describe strategies to enhance teamwork.
  • Strengthen the CHW Coalition in New Hampshire.

Target Audience: Community Health Workers; representatives of health systems, social service, and community organizations; and legislators who seek new models of care to promote access to services and minimize health disparities.
