Waiver and Release of Liability Relating to Coronavirus/COVID-19

The coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is reported to be extremely contagious and is believed to spread from person-to-person contact and/or by contact with contaminated surfaces and objects, and even possibly in the air.

I understand that attendance at the Bigger, Lighter, Stronger: 2021 IBC Update for Exterior Adhered Porcelain Facades put on by the Chicago Chapter of Construction Specifications Institute at the East Bank Club on September 28, 2021, in Chicago, Illinois, includes possible exposure to and illness from infectious diseases, including COVID-19. 

Therefore, in consideration of being permitted to attend the Bigger, Lighter, Stronger: 2021 IBC Update for Exterior Adhered Porcelain Facades, I acknowledge the contagious nature of COVID-19 and the risk of injury that may result from exposure or infection to the same. I voluntarily agree to assume these risks and accept sole responsibility for any injury (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with my attendance at The IBC Update for Exterior Adhered Porcelain Facades relating to COVID-19 ("Claims").

On my behalf, I hereby release, covenant not to sue, discharge, and hold harmless the Construction Specifications Institute, its chapters, employees, agents, volunteers, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating to any exposure or infection with COVID-19.  I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of the Construction Specifications Institute, its chapters, employees, agents, volunteers, and representatives, whether a COVID-19 infection occurs before, during, or after attending the Bigger, Lighter, Stronger: 2021 IBC Update for Exterior Adhered Porcelain Facades.

I further expressly agree that the foregoing assumption of risk, and release and waiver of liability is intended to be as broad and inclusive as is permitted by the laws of the State of Illinois and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect. I understand and agree that this release and waiver of liability is governed by the laws of the State of Illinois.   

I have carefully read, I fully understand, and voluntarily sign this waiver and release of liability, and further agree that no oral representations, statements or inducement apart from the foregoing written agreement has been made.
