CSI Chicago's Holiday Party and Fundraiser 

It’s the holiday season which means it’s time to spread holiday cheer and fundraise for the CSI Chicago Foundation. Please join us December 15th at the East Bank Club for a good cause. 

Event Guidelines: Attendees must agree to this COVID-19 waiver and wear a mask at the event in accordance with local guidance and adhere to IDPH, City of Chicago, and East Bank Club guidelines.

Waiver and Release of Liability Relating to Coronavirus/COVID-19

The coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is reported to be extremely contagious and is believed to spread from person-to-person contact and/or by contact with contaminated surfaces and objects, and even possibly in the air.

I understand that attendance at the Holiday Party put on by the Chicago Chapter of Construction Specifications Institute at the East Bank Club on December 15, 2021, in Chicago, Illinois, includes possible exposure to and illness from infectious diseases, including COVID-19. 

Therefore, in consideration of being permitted to attend Words Matter: The Secret Life of Specifications, I acknowledge the contagious nature of COVID-19 and the risk of injury that may result from exposure or infection to the same. I voluntarily agree to assume these risks and accept sole responsibility for any injury (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with my attendance at the Holiday Party.

On my behalf, I hereby release, covenant not to sue, discharge, and hold harmless the Construction Specifications Institute, its chapters, employees, agents, volunteers, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating to any exposure or infection with COVID-19.  I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of the Construction Specifications Institute, its chapters, employees, agents, volunteers, and representatives, whether a COVID-19 infection occurs before, during, or after attending the Holiday Party.

I further expressly agree that the foregoing assumption of risk, and release and waiver of liability is intended to be as broad and inclusive as is permitted by the laws of the State of Illinois and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect. I understand and agree that this release and waiver of liability is governed by the laws of the State of Illinois.   

I have carefully read, I fully understand, and voluntarily sign this waiver and release of liability, and further agree that no oral representations, statements or inducement apart from the foregoing written agreement has been made.
