United Hospice, Inc. presents the "Let's Dance Gala"
We are thrilled to host our 2021 Let's Dance Gala on Sunday, October 24, 2021 For the safety and health of our Gala guests, proof of COVID-19 vaccine or negative PCR Covid test obtained 3 days before the event will be required.
Please submit proof of vaccination or negative test using the form below by October 21, 2021.
Masks and hand sanitizer will be provided to each guest upon entry to the event. Guests will be required to wear a mask throughout the event except when eating or drinking.
To ensure the privacy of our guests, all information relating to the vaccination status will be permanently deleted and/or destroyed directly following the Gala event.
