Food Truck Application Form- Witches Ball October 6, 2018 

APPLICATION DEADLINE is August 15, 2018. 

Date of the event is Saturday, October 6, 2018 Rain or Shine.

This year, due to circumstances beyond our control, the event has been split into two events, the family-friendly Pumpkin Ball running from 3pm to 6pm and the Witches Ball, for attendees aged 21 and over, running from 7pm to Midnight.  

You will have the choice to apply for one or the other, not both.

Fees:  $250.00 This includes your $100 1 day permit fee due to the township, due upon acceptance.  Requirements for this permit will be collected prior to the event and submitted on your behalf. Permits will be handed out the day of the event and must be displayed on your truck.

The Township of Mt. Holly requires each vendor to obtain a $50 individual one-day Vendor Permit.  Your booth fee includes the permit fee!  We will apply for the permit on your behalf once you have been approved to vend.  Because of this permit requirement, the application deadline will not be extended.

We will also require the following documents upon acceptance:

Current Burliington County Health Department Health Certificate – a written approval and current inspection from the Burlington County Health Department MUST be obtained prior to the event.  This report must be prominently displayed during the event.   For more information about food inspections, contact 609-265-5521 ext. 5521.  A copy of this will be required by event management for the vending permit application also.

Certificate of Insurance -  Mill Race Village Arts & Preservation, Mill Race, Inc, Mount Holly Township, and Winzinger, Inc.  assume no responsibility for loss or damage to the food truck, it’s property, employees or guests.  Each food truck vendor must supply individual Property & Liability Insurance naming Mill Race Village Arts & Preservation, Mill Race, Inc, Mount Holly Township, and Winzinger, Inc. as additionally insured.

This application does NOT guarantee a space.  All applicants will receive a notice of either acceptance or rejection by August 31, 2017.  Booth fee of $150 is due upon acceptance.

 GENERAL INFORMATION:

The Witches Ball in Mount Holly is 17 years old this year.  It has grown and transformed over the years from a tiny local event in Mill Race Village featuring ballroom dancing witches to a celebration that draws people from all over the tri-state area and beyond to enjoy music, food, performances, art, unique shopping and outrageous costumes.  

 With growth, come growth pains.  Due to circumstances beyond our control we have had to make some radical, but hopefully positive, changes to better serve our audience.  It will all still be free and open to the public, but the format has changed.

 3pm-6pm The Pumpkin Ball– family-friendly event, with activities for kids and their parents, costume contest and more.

 3pm-8pm The Bizarre Bazaar– vendors of the exotic and unusual. 

 7pm-Midnight  – The Witches Ball  - Adults only, 21 and over, gated event with all of what you have come to expect from The Witches Ball; music, performances, costume contest, Beer Garden, Voodoo Bar and more.

First Aid:  Each food truck must have an approved fire extinguisher and a first aid kit in the truck at all times

Change: Cash drawers are the responsibility of each vendor

Alcohol:  No alcoholic beverages may be sold or served by the food truck vendors or an employee of the food truck vendor

Placement of the food trucks will be at the discretion of the event management team.
