Mastering Clinical Documentation

Documentation is typically the least favorite part of any clinician’s daily tasks.  However, it is a very significant job function that is often scrutinized by employers, regulatory entities and even, potentially, in court.  This workshop offers clinicians an overview of the legal aspects of documentation, including HIPAA & 42CFR
Part 2 compliance.  Discussion will focus on assessments, treatment plans and progress notes, to name a few.  Participants will learn about client-centered language and the benefits of writing succinctly through written practice activities.  Course content emphasizes the concept of “If it isn’t written, it didn’t happen”.

Upon completion of this course, participants will be able to:

  • Identify and discuss the legal aspects of documentation and electronic recordkeeping in practice.
  • Identify and apply critical components of assessments, treatment plans and progress notes.
  • Incorporate the “Golden Thread” throughout documentation.
  • Summarize an individualized and person-centered approach to clinical documentation.

Presenter:  Ana Guerra, LCSW, LPC, LCADC, CCS
Ms. Guerra, is a seasoned mental health professional with extensive experience in mental health and substance use disorders. She specializes in the treatment of Co-Occurring Disorders and Trauma. Ms. Guerra parlayed her clinical experience into a career as an adjunct/faculty member for various colleges and universities in NJ. She proposed, developed the curriculum and taught the Substance Use Disorder courses at Kean University’s Social Work Department. Ms. Guerra teaches across the Bachelor and Master level curriculum in Social Work and Addiction Studies including courses such as “Trauma” and “Psychopathology”. Ms. Guerra is approved by the Addiction Professionals Certification Board of NJ (APCBNJ) to teach initial and re-certification L/CADC courses. As a trainer for New Jersey Prevention Network, she served as an integral team member revising the CADC curriculum. Ms. Guerra facilitates training and consultation for various mental health, substance use and academic settings. She maintains a private practice and is also a Clinical Supervisor for a residential and outpatient treatment facility. In 2019, Ms. Guerra was selected by the Council on Social Work Education (CSWE) to participate in the development of the CSWE’s Substance Use Curricular Guide which was published in 2020. Ms. Guerra was approved by the APCBNJ as well as the NJ Alcohol and Drug Counselor Committee as a Mandatory Legal Class Trainer in 2021 and 2022, respectively.

This course Mastering Clinical Documentationand the instructor, Ana Guerra have been approved by the Addiction Professionals Certification Board of NJ.  This course, provided by Behavioral Health Matters, approval #403092122REC4 has been approved for 4 continuing education credit hours by the APCBNJ.  Credentials issued by the APCBNJ, the NJ Board of Marriage and Family Therapy Examiners and the NJ Board of Social Work Examiners recognize such courses.  This 4 CE course is approved for social work continuing education hours in accordance with N.J.A.C. 13:44G-6.4c12.

NY Social Workers:
