Nurtition and Complex Care

Homestyle Direct in conjunction with Direction Home of Eastern Ohio is proud to offer a unique CEU program customized to address the complex care management of people who have health conditions that are impacted by medically tailored nutrition.  This 1-hour CEU program will be conducted by our Registered Dietitians who have experience in helping to address food and nutrition security with those who have diabetes, cardiac, renal and behavioral health challenges.

• The program will focus on trends in healthcare related to diabetes, cardiac renal and behavioral health and health equity challenges.

•The program content will review how people are managed with these conditions through a complex care model to help control costs while leading to positive outcomes

• Medically tailored nutrition needs of these populations will also be addressed with specific attention to the nutrient guidelines important to help control diabetes, cardiac, renal and behavioral health conditions.

• The instructors will also provide care management teams with resources for community-based nutrition options and education to help support positive outcomes for those who face nutrition security.

Continuing Education 1.0 Hour approved for:

LSW/LISW/SWA/LPC/LPCC

Registered Dieticians

Refunds will be made ONLY for cancellations received three business days prior to the event. If received less than three days prior to the event, a credit can be issued for use at another program within the next 6 months. If notice of cancellation is not received before the date of the program, no refund will be issued.  PLEASE NOTE: No shows will be charged the full registration fee.

Class sizes are limited, so register early. Classes with low registrations may be canceled. If the class is canceled or moved, participants will be contacted via email at least 24 hours in advance.

Classes start and end on time. To ensure a quality experience for all, please log in at least 15 minutes prior to the class to resolve any technical issues.  CEU credits will be adjusted 0.25 credit hours for every 15 minutes not in attendance; adjusted certificates will be emailed to participants within 3 business days after the training. Hard copies of certificates will be mailed upon request.

The webinar CEUs will take place on Zoom during the designated times. The link to join the webinar session will be sent in your registration confirmation email. 

Please note: Certificates will be sent electronically so long as the virtual attendance has been verified and evaluation returned. A reminder that, in keeping with licensure board requirements, certificates will be adjusted 0.25 hours for every 15 minutes not in attendance, and attendance will be monitored.
