STEP FOUR - References & Signature

STEP FOUR...you're almost finished!  In order to apply for MedQuest Health Careers Camp, we require that you submit the name, e-mail address and phone number of two references.  The first reference must be a teacher.  The second reference must be a health professional, school counselor, community leader, minister or priest.  

In STEP FOUR you will be asked how you intend on paying the $50 deposit.  If you are paying the deposit with a credit card, you will be taken to the PayPal site to enter the payment information.  The payment will post to your account immediately.  If you elect to send the $50 deposit in via check or money order, instructions will be displayed on the screen.  Your application is not complete until the $50 deposit has been recieved.  Your deposit must be recieved by April 15, 2012.

STEP FOUR also requires an electronic signature from you, the applicant.  If you are under the age of 18, your parent or guardian must sign as well.  By completing the electronic signature, you certify that everything on the application is true and correct.
