Connecticut Patient Navigation Conference
The Connecticut Cancer Partnership in collaboration with the Connecticut Department of Public Health, the American Cancer Society and the Leukemia and Lymphoma Society is hosting a one-day conference on Patient Navigation.
The goals of the conference are:
- to enhance professional development of patient navigators and supervisors,
- to promote collaboration, networking, and promote the culture of patient–centered care in Connecticut health care settings, and
- to provide tools for navigation stakeholders in the development, implementation, execution and evaluation phases of patient navigation programs.
In addition, the conference will enable participants to recognize that the field of patient navigation is still developing and different/competing models are emerging and finally, to identify advantages and disadvantages of different models within the context of different settings.
Attendees can receive 4.0 CEUs for their participation.
Because there are many different models of Patient Navigation programs across the state, the Partnership identified a definition for "patient navigator" as outlined by the American College of Surgeons Commission on Cancer to mean: "a trained, culturally sensitive healthcare worker who provides support and guidance through the cancer care continuum; whose duties include helping patients to "navigate" through the maze of doctors' offices, clinics, hospitals, outpatient centers, insurance and payment systems, patient support organizations and other components of the healthcare system".
The conference is free but registration is required. Registration closes on March 30, 2012 at 4 PM.

1A. Interdisciplinary case study
- Audience: Navigators, Nurses, Social Workers, Case Managers and other clinical staff
- Description: This session will use case studies to illustrate how a Navigator can determine and then eliminate barriers: including access issues for disparate populations, and problems caused by gaps between different clinical facilities. The session will also address the value of Patient Navigation in patient centered medical home requirements. The case-study approach will highlight the continuum of care and showcase successful team work.
1B.Why be a Champion for Patient Navigation
- Audience: Navigation Supervisors, Program Developers, Hospital Boards, Insurance Executives, Cancer Committees and other administration positions
- Description: This session will discuss the benefits of creating a Patient Navigation Program and how community benefits assessments reveal how Navigation Programs can be an asset to Cancer Centers regarding patient satisfaction, reimbursement rates, compliance, meeting CoC accreditation standards and preventing outpatient migration.

2A. Models of Navigation (Panel Presentation)
- Audience: Navigation Supervisors, Program Developers, Hospital Boards, Insurance Executives, Cancer Committees and other administration positions
- Description: This session will allow different models of Patient Navigation to be presented through a targeted question and answer forum. Topics to be covered include: budgetary considerations, reporting structure, and effective workflows of a care team that utilizes Patient Navigators. There will be time for an audience Q & A period towards the end of the session.
2B. Integration of Quality Care throughout the Cancer Experience
- Audience: Navigators, Nurses, Social Workers, Case Managers and other clinical staff
- Description: This session will cover outcomes associated with symptom relief, communications with patients and families, as well as transitions of care, for example: when to let go of a patient, discussing hospice or alternative treatment options.
1:30 – 3:15 - Three- Part Plenary Session: Measuring Effectiveness
- Part I: This plenary will discuss different methods and tools used to measure Patient Navigation Programs and how the data collected can be of use to Cancer Centers. The session will also discuss how metrics should be determined and collected to measure specific goals and objectives.
