This form is required for many activities and events sponsored by Carmel UMC and is helpful for keeping participants safe by having current medical and insurance information on hand in case of an emergency. In order to keep this information up-to-date, you will expected to fill this out at least once per year (or you can opt to fill it more often). We also ask that you contact us if you have any changes to your personal, medical or insurance information so we can update our records.
