from Adolescence to Adulthood:
Children with & without Special Health Care Needs

OVERVIEW

Data from both national and local studies are clear that pediatricians do not start the transition process early enough or in a formalized way.

Young adults with and without special health care needs are frequently “lost in transition” and utilize emergency services for their primary care, or forego care completely until an urgent situation occurs. The lack of primary health care and preventive health services ultimately leads to poorer health outcomes and avoidable increases in health care costs.

At the conclusion of this conference participants will be familiar with the following:

  • The importance of an organized transition process for all adolescents.
  • The essential elements of a transition process from the perspective of primary care pediatrics, pediatric and internal medicine subspecialties, primary care internal medicine/family medicine.
  • Available tools to assist health care providers and families in the transition process.
  • Legal aspects of transition.
  • Housing, education, health insurance, and employment options for adolescents with special needs.
  • The concept of a formal transition policy for every practice, as well as a completed policy draft for their own workplace.
  • Understanding roadblocks to smooth transition and how to overcome them.
