2014 Continuous Quality Improvement Summit

About the Summit
Join us for a two-day interactive training session that will focus on building strong quality improvement infrastructure in our changing health care environment.   Participants will have the opportunity to network with colleagues from across the DC metropolitan area and will leave with practical tools and resources that will drive quality improvement strategies. **Program participants can also earn CMEs and CEUs**

Dynamic sessions will cover:

• Care Coordination
• Care Transition
• Continuity of Care
• Population Health Management (use of registries, empanelment)
• Patient Engagement (motivational interviewing)
• PCMH and Beyond (review new proposed standards)
• Data Management (including use of dashboard, data analysis, run charts)
• Implementing QI Principles
• Maximing your EHR (using technology to enhance health outcomes)
