8:45 to 10:00 a.m. - Assess/analyze "Quality Improvement Tool For Review of Acute Care Transfers" for reasons/causes of readmissions

10:00 to 10:15 a.m. - Break

10:15 to 11:30 a.m. - Select the appropriate tools for implementing to eliminate/reduce the identified reasons/causes for readmissions

11:30 to 12:15 p.m. - Identify methods for implementing appropriate tools for success

12:15 to 12:30 p.m. - Questions and answers & evaluation

Goal: To assist nursing homes in reducing unnecessary transfers and readmissions through increased use of INTERACT tools.

Objectives 

• Assess reasons for/causes for readmissions to hospitals
• Select appropriate INTERACT tools for implementation to reduce readmissions
• Identify methods for implementing appropriate tools

Target Audience: Administrators, DONs, staff involved with quality and/or readmissions, and corporate staff
