• North Country Health Compass Partners: Improving Population Health in Jefferson, Lewis and St. Lawrence Counties (Population Health Track) - The North Country Health Compass Partners are a 22-agency collaborative focused on improving health for all residents of the region. They have spearheaded the development of a regional health assessment, health improvement plan and health information website. This session is designed to share best practices an review their engagement process.

• Organizational Capacity-Building: Planning that Reaches Beyond Growth for Long-Term Stability (Organizational Track) - Participants in this workshop will discuss the typical lifecycle of a nonprofit organization, review the Four Core Capacity Model for organizational capacity building, identify benchmarks for their own organization, and then be able to utilize tools and techniques to develop the framework for their own successful capacity building plan.

• Opportunities for Telemedicine in Northern New York (Clinical Track) - This session will describe the infrastructure in NNY to create a telehealth center of excellence. It will provide attendees with an overview of the North Country Healthcare Provider's (in partnership with the Fort Drum Regional Health Planning Organization and the Adirondack Health Institute) role in bridging 2 fiber networks for the deployment of telemedicine, VoIP and professional development opportunities. 
  
  Telehealth presents a unique opportunity in our rural region to address transportation and workforce challenges. This presentation will highlight telemedicine reimbursement, regulatory barriers in NYS, as well as lessons learned from growing a network consisting of 7 rural counties. We will specifically discuss a pilot study to increase access to child and adult psychiatry

• Advancing New York State’s Prevention Agenda: Nontraditional Approaches to Improve Population Health  (Population Health Track) - In March 2014, the New York State Health Foundation (NYSHealth) awarded 17 grants of up to $50,000 to assist 28 counties with their CHIPs. For this session, NYSHealth will identify two local health departments to co-present on CHIP strategies for success and barriers to implementation.

• Regional Health Planning: Transforming Healthcare Together through Community Collaboration and Partnership (Organizational Track) - Finger Lakes Health Systems Agency brings focus to community health issues through data analysis, community engagement and solution implementation through community collaboration and partnership.

• Ellenville Regional Community Medication Safety Project (Clinical Track) - The process we experienced from 2008 to 2014 developing a community wide approach improving medication safety and reducing medication misadventure. Problem recognition, development and implementation of solutions are presented with time for questions.

• Towards Health Equity: Engaging Rural Residents in the Creation of Opportunities for Healthy Living (Population Health Track) - Striving for best practice in medicine and achieving the quadruple aim involves continuous change to meet the needs of our patients and our communities.  This session will provide you with action specific, evidence based solutions to implementing this model of care and addressing the barriers so often present in rural communities today.

• Overcoming Barriers to Healthy and Sustainable Outcomes (Organizational Track) - This presentation examines obstacles to healthcare as experienced by those affected by homelessness in a rural geography—and how policy and practice strategies can improve overall outcomes. It discusses what is known about rural homelessness and unstably housed people and how this differs from their more urban counterparts. Content focuses on the relationship between rural homelessness and health, and the importance of healthcare as a vehicle to establish sustainable outcomes for homeless and at-risk rural populations. The content highlights best practice models of policy and practice to improve service access and reduce long term medical issues as well as the incidence of rural homelessness.

• Transformation of Rural Primary Care: Patient Centered Team-Based Approach Integrating Care Coordination, Care Management Quality, and Performance Improvement (Clinical Track) - This session will describe an innovative model for community health improvement that privileges residents of rural communities as co-producers of health. The work and achievements of the Dundee Health Improvement Project will be featured.
