Seeing the Big Picture: Social Determinants of Health and Health Links

The South East LHIN invites health care providers to come together at the 2014 Primary Health Care Forum for a day of learning and networking  on the theme "Seeing the Big Picture: Social Determinants of Health and Health Links".   Registration  to the Forum is free but space is limited.

Red Code Project
CODE RED is an ongoing project started in 2010 by The Hamilton Spectator to explore the powerful impacts that the social determinants of health have on people in Hamilton.

Using data measuring a wide range of health, social and economic variables, CODE RED has been exposing disparities at the neighbourhood level in Hamilton through stories and maps 

Poverty Screening Tool

Developed by the Poverty & Health Committee of the Ontario College of Family Physicians,  the ‘Clinical Poverty Tool’ features the ‘screen, adjust risk, treat’ paradigm used to treat income as a social determinant of health. The plenary session will explore partnerships and local adaptations spawned by the tool, as well as opportunities for its use in the South East LHIN

Stumbling Upon a New Model for General Practice

A major dilemma for health and social care services around the world is how to support the management of long term conditions in the context of rising demand and costs.
Our learning from 3 prototypes and a further 32 innovation sites in General Practice in the UK has led us to stumble across a new model for general practice which is designed to meet these challenges.  In this model citizens and patients play a pivotal role in the promotion of health and well-being, working closely with General Practice.

Kingston Health Link will share lessons learned on implementing a care delivery model that now includes a community social services navigator for complex patients.

Facilitating Behaviour Change in the Health Links Setting: Developing and Enhancing Self-management Support Skills and Communication Techniques to Nurture Client Change

Participants will understand the evidence based process of WrapAround which trains community members (family faith and friends) to facilitate strength based plans that keep seniors at home.
