Course Abstract:

Treatment alone often fails people. Evidence based statistics prove this over and over again. The 5 year plan breaks down all key components to a successful recovery. The five  year  plan was created after learning about long term diversion programs. This presentation goes in depth to explain how we build a seamless continuum of care morning past a 28 day model.

Course Objective:

(1) Show  evidence  based statistics.

(2) Outline a seamless continuum of care.

(3) Breakdown the components of and benefits of a 5 year plan.
