This month we will have Lisa Yorra, marketing director at Elder Service Plan (ESP) of Harbor Health Services, Inc., and Alexandra Schweitzer, program director of Senior Care Options (SCO) from Tufts Health Plan in Mass, present on their services and answer any questions you may have. 

Both the PACE and SCO programs use both Medicare and MassHealth finding to provide comprehensive, community -based care, all coordinated by a care manager for every member. You'll learn about eligibility, benefits, and care management that leverages community-based and medical services to keep people safely in the community, as well as the differences between the two programs.
