Saturday and Sunday,  March 21 and 22, 2015:  8 AM - 5 PM

Cost of Course includes lunch and the books: Introduction to the FDM and Why Does It Hurt?
Physicians and approved mid-level providers with physician sponsor - $675      
Medical Students $100 

Cancellation Policy:  Full Refund on or before 3/15/2015, 50% Refund 3/16/2015 - 3/19/2015. 
No refund after 3/19/2015.

The American FDM Association is accredited by the American Osteopathic Association to sponsor CME for physicians.  Credit will be applied for through both AOA and AAFP and 19-20 hours are expected.

Application for CME credit has been filed with the American Academy of Family Physicians. Determination of credit is pending.

Physicians should claim only the credit commensurate with the extent of their participation in the activity.
