The Physician Form needs to be completed by your child's pediatrician prior to class. It is required for admittance to the class. Please download from our website and send it in or bring it with you to the first night of class.

Joanna Stuck is a licensed Physical Therapist and Certified Infant Massage Instructor. She brings a wealth of experience to the Easter Seals team of professionals with her experience in pediatric therapy and as Director of a therapy clinic. She also provides leadership in her moms group, preschool board, and with the Angels Arms ministry for families with children with special needs.
