Population Health in Oregon Forum
The healthcare industry is continuing its evolution from a fee-for-service model to a quality of care model. This trend will only accelerate after the announcement earlier this year by the Department of Health and Human Services plan to tie 50% of Medicare provider payments to value-based models. This enormous shift in care and payment models is helping to drive the growth of population health management. Through the promise of population health, hospitals plan to deliver better care by engaging the patient earlier, providing better care quality, reducing wasteful tests and treatments and enabling better coordinated care.
A critical leg of delivering this value-based care will be the hospitals' infrastructure. Electronic Medical Records are enabling providers to capture new data in pursuit of better patient care but organizations will need to be able to collect and synthesize large amounts of data into actionable improvements in care delivery.
In Oregon Asante, Bay Area Hospital, Mid-Columbia Medical Center, Moda Health, Oregon Health & Science University, Salem Health, Sky Lakes Medical Center, and St. Charles Health System came together and founded Propel Health. By aligning providers, patients and payers, Propel Health offers the tools, technology, and resources to integrate care delivery and enhance the patient experience.

Moderator: Richard Gibson, Research Director in Healthcare, Gartner, Inc
Richard Gibson, MD, PhD, is a Research Director in Healthcare for Gartner, Inc. He is an Affiliate Assistant Professor in the Department of Medical Informatics and Clinical Epidemiology at Oregon Health and Science University. Formerly he was Chief of Healthcare Intelligence at Providence Health & Services, Renton WA. Previously he was Senior Vice President and Chief Information Officer at Legacy Health in Portland OR. Dr. Gibson is a family physician, emergency physician, and also boarded in Clinical Informatics.
Melissa Davies, Director of Technology & Analytics, Propel Health
Melissa brings with her over 10 years of healthcare operations experience both within and as a consultant for large health systems. She has had demonstrable outcomes in project management, process improvement, change leadership, operational implementations, data and analytics capability development, revenue cycle improvements, and in financial modeling and budgets. Melissa is a proud Alumni of Boise State University having received her Master’s in Instructional & Performance Technology from the College of Engineering following the completion of her undergraduate degree in Health Informatics & Information Management, with a Business Minor, from the College of Health Sciences.
Dr. Lee Milligan, Medical Director of Informatics (MDI), Asante Health System
Dr. Milligan completed his undergraduate studies at the University of Utah and medical school at George Washington University in 1997. He went on to train in Emergency Medicine at UCLA-Harbor and graduated in 2000. He has practiced as a board certified Emergency Medicine physician at Asante in southern Oregon since that time. He is a Certified Trainer and Certified Physician Builder within the Epic EHR system. He continues to actively build within Epic and has been featured on Epic Earth with a build that is designed to assist physicians during in-hospital cardiopulmonary arrest [CodeRunner]. As Medical Director of Informatics for Asante he oversees the Physician Informatics program.
Within Asante, he currently serves as Chair of Asante’s PAC (Physician Advisory Council) and APAC (Ambulatory Physician Advisory Council) and is a member of the Asante Physician Partner’s board of directors. In addition, he is directing Asante’s medical Tele-Health strategy, chairs their Health Maintenance workgroup, is leading their mobile Apps implementation (Epic’s Haiku/Canto), directs their IMO clinical interface terminology engine, and is Medical Director of both ICD-10 and their CDI program. He is also a member of the Data Governance Management Team, Epic Coordination Committee, Patient Experience Committee and Primary Care Advisory Committee.
Outside of Asante, for Propel Health, he serves as co-chair of the 14-member Quality Health Management Committee, leads their IT Work stream efforts and he serves on Propel Health’s Board of Directors. At the OHLC’s EDIE initiative, he serves on both the Analytics and Operations committee.
Dr. Stephanie Halvorsen, Clinical Associate Professor of Medicine, Division of Hospital Medicine and Medical Director for Clinical Integration, OHSU
Dr. Halvorson is an Internal Medicine Hospitalist and a member of the teaching faculty at Oregon Health & Science University (OHSU). In addition to her clinical work, Dr. Halvorson serves as a Medical Director for the Department of Clinical Integration at OHSU. Clinical Integration is a new and innovative approach to clinical care that uses data supported, best practice methods and community-provider connections to streamline care delivery and increase value to individual patients and patient populations. She is passionate about patient care and reforming systems of care and care delivery to better serve patients. Dr. Halvorson also serves on the Quality Health Management committee, which is the physician-governance committee for Propel Health.
