Mitigation Strategies to Overcome Risks in Transitions of Pediatric Patient Care

The pediatric population is at high risk for medication errors, especially during transitions of care. Due to their unique needs and complex medication regimens, pediatric patients necessitate special consideration when prescribing and managing their medications.  Improving medication management across the continuum of care requires a collaborative team approach among physicians, nurses, pharmacists, and the pediatric patient as well as family. This activity will describe a medication error related to transition of patient care and discuss medication-related vulnerabilities in transitions of care, with the purpose of identifying mitigation strategies to reduce or eliminate potential medication errors and improve patient safety.

Learning Objectives:
1. List two risk factors that may cause patient harm in the topic discussed.
2. List two potential interventions that may prevent patient harm.
3. Describe the role of pharmacist in optimizing medication safety.
