Horizon's 14th Annual Golf Outing and Banquet - Sponsor 

Sponsorship Levels:

Review the five sponsorship levels below and choose the one that works best for your company. Please register by May 15, 2016 to reserve your sponsorship.

ACE - $1,500 Title Sponsor:

Your company name and/or logo will be printed on a banner at the outing and at our annual summer camp program. In addition, your company will receive all the benefits of the Double Eagle Sponsorship.

DOUBLE EAGLE - $1,000 T-Shirt Sponsor/Foursome:

Your company name and/or logo will be recognized on Horizon’s official camp shirt worn by people across the Midwest, one foursome with VIP treatment, including easy check in. Company recognition at awards ceremony held during the banquet. In addition, your company will receive all the benefits of the Eagle Sponsorship (*except the Ad Specialty Item)

EAGLE - $500 Exclusive Hole-Sponsor/Ad Specialty Item Sponsor:

Your company name and/or logo will be on the only sign at one of the holes.  Any company promotional items you provide will be included in our golfer goodie bags. In addition, your company will receive all the benefits of the Birdie Sponsorship.  (*Except webpage and emails will state “Exclusive Hole-Sponsor.”)

BIRDIE - $250 Hole-Sponsor:

Your company name and/or logo will be printed on a sign at one hole.   Your company name and/or logo will also be recognized on our golf webpage as a hole-sponsor.  Your company will also be recognized as a hole-sponsor to over 1200 subscribers via our July e-newsletter and all golf outing e-mails. Our e-mail newsletters are distributed nationwide and read by a variety of professionals.  In addition, your company will receive all the benefits of the Par Sponsorship.

PAR - $100 Program Sponsor:

Your company name and/or logo will be recognized in our event program handed out to all participants.It's a special year for the Association of Horizon, Inc. as we are celebrating our 25th year of hosting a week-long summer camp for adults with physical disabilities!
