Saturday and Sunday,  February 27 & 28:  8 AM - 5 PM

Course Fees
Cost of Course includes lunch and the books: Introduction to the FDM and Why Does It Hurt? 
AFDMA Member  - $700
Residents - $450
Active Military - $600
Medical Students (DO/MD) - $100
Nonmembers - $950  (or consider joining AFDMA when you register and save $50 today!) 

Currently registration is only open to the following registrants:
15 physician or midlevel providers
20 medical students (D.O/M.D) 

A waiting list will be created once these limits have been reached.

Cancellation Policy 

Full Refund on or before 02/05/2016, 50% Refund 02/05/2016 - 02/19/2016. 
No refund after 02/19/2015. However, course fee can be applied to future course.

Hotel Reservations
AFDMA has not established a room block in Anchorage allowing registrants the opportunity to stay anywhere. The recommended hotels are the Best Western Golden Lion Inn at 1000 E 36th Ave, Anchorage, AK 99508 and is located across the street  from the meeting site. Call  (907) 561-1522 to make a reservation. The other hotel to consider is the Marriot Residence Inn Anchorage Midtown at 1025 E 35th Ave, Anchorage, AK 99508 and is located one block away from the meeting site. CME Hours
The AFDMA has requested that the AOA Council on Continuing Medical Education approve this program for 20 credits of AOA Category 1A CME credits.  Approval is currently pending.

This AFDMA program has been approved by the American Academy of Family Physicians for 19.00 Prescribed credit(s).

All other healthcare professionals must submit their seminar certificate to their licensing organization to receive CEUs.

Physicians should claim only the credit commensurate with the extent of their participation in the activity.
