Spring 2016 Locker Requests 

This form is for requesting a locker at the UCA for instrument storage only.  IF YOU ARE LOOKING FOR THE KEYCARD ACCESS REQUEST FORM, PLEASE CLICK HERE.

Lockers are now available for the primary instruments of all students who are registered music majors and for all minors who are enrolled in applied lessons, free of charge.  Lockers will be administered on a first-come, first-serve basis. Students who are eligible for a free locker must request a locker by Monday, February 8 in order to be guaranteed a locker for the Spring semester of 2016.   Students may be assigned a shared locker, based on availability.  Please note we only provide a free locker for your primary instrument (the one you take applied lessons on). 

We will reserve a number of lockers at the beginning of the semester for non-majors requesting a locker.  When these fill up, any remaining requests will be put on a wait-list until February 8.  These students will be charged a $35/semester fee for using the lockers, which will be billed for each individual semester of use.

After the third week of classes (February 8), lockers will be released for use other non-major students enrolled in ensembles or for requests for secondary instruments from majors or minors enrolled in applied lessons.  If you are requesting a locker for a secondary instrument at this time, please go to the front office to make this request.  You do not need to fill out this form again.

Locker Rates (applicable to students who are not majors or minors, or are not enrolled in applied lessons during the current semester)

Small/Book Lockers: $17.50/semester
Shared Instrument Locker: $17.50/semester
Individual Instrument Locker: $35.00/semester

If you are planning to rent an instrument from CSU, it will be assigned to its own locker and you DO NOT need to fill out this form.  All CSU instruments will be assigned a locker behind the Organ Recital Hall upon rental.

We do not accept requests for specific locker assignments, and once a locker has been checked in, it is put back into the overall pool of available lockers for redistribution by the department.

CSU maintains zero liability for any objects stored within the lockers - it is your responsibility to insure your property in the event of theft, damage, natural disaster, etc.

Please note that once you are assigned a locker, you will have access to that locker until the end of the Spring semester of 2016 at which point you will be required to return both your lock and the Locker Check-In Form to the MTD front office.  Lockers are available for use during the summer semester by special request only for a $35 fee (contact Sandra Sanchez at to make this request).

All lockers must be turned in to the main office by the last day of finals in the spring semester (Friday, May 15).
