Congratulations! We are excited to meet you in Orlando and to have you join your fellow FDM experts as you move forward to your international certification.

The IC Examination is available to AFDMA members only. Verification of membership will be made once registration is received, or please contact Holly Macriss, AFDMA Executive Director, at afdmausa@gmail.com to check on your membership status.

Registration is only open to those individuals who have completed all three (3) Modules taking in the US and taught by AFDMA instructors and have had one (1) year of practice following completion of Module 3. Verification will be made and if there are any discrepences, we will contact you.

Course Fees
Advanced registration is required. No onsite registration will be available.
AFDMA Member  - $300

Cancellation Policy 
Full Refund on or before 02/15/2016, 50% Refund 02/16/2016 - 02/29/2016.  
No refund after 02/29/2015. However, course fee can be applied to future course.

Location
This examination takes place in conjunction with AAO convocation in Orlando, Florida. We will notify you of the specific location closer to the date.

Hotel Reservations
AFDMA has not established a room block in Orlando allowing registrants the opportunity to stay anywhere.

CME Hours
No CME hours are offered for this examination.

Examination Schedule

Day begins with written examination, then video interpretation, followed by practical examination with three instructors. Be prepared to work on instructors as well as fellow examinees.

Results of the exam will be available that day.

Breaks to be inserted as needed. Lunch will be on your own.
