BY REGISTERING FOR THIS EVENT, YOU AGREE TO THE FOLLOWING:

I hereby grant absolute right and permission to the Massachusetts Black Lawyers Association, its legal representatives and assigns, hereafter referred to as the “MBLA,” in connection with its 2015 Know Your RIghts: Call To Action program on February 26-27, 2016, to take, use, reuse, publish, and republish photographic portraits or pictures of me and/or the minor named below or in which I or the minor may be included, in whole or in part.  I also consent to the use of any published matter in conjunction with such photographs, including in the MBLA’s print, online and video-based marketing materials, as well as other MBLA publications.. I specifically consent to the digital compositing or distortion of the portraits or pictures, including without restriction any changes or alterations as to color, size, shape, perspective, context, foreground or background. I waive any right that I or the minor may have to inspect or approve any finished product or products or the advertising copy or printed matter that may be used in connection with such photographs or the use to which it may be applied.

I hereby release and hold harmless the MBLA from any reasonable expectation of privacy or confidentiality associated with the images specified above.  I further acknowledge that my participation is voluntary and that I will not receive financial compensation of any type associated with the taking or publication of these photographs or participation in the MBLA marketing materials or other MBLA publications. I acknowledge and agree that publication of said photos confers no rights of ownership or royalties whatsoever.

 I hereby release the MBLA, its contractors, its director, officers, employees, and any third parties involved in the creation or publication of marketing materials, from liability for any claims by me or any third party in connection with my participation.

ALL MINORS MUST SUBMIT A SIGNED RELEASE FORM TO INFO@MASSBLACKLAWYERS.ORG BY FEBRUARY 25, 2016.
