Learning Objectives
1) Review the six distortions – their etiology and progression
2) Instruct participants on the use of the FDM to treat conditions in the upper extremities such as 
tennis elbow and carpal tunnel.
3) Instruct participants on the use of the FDM to treat conditions in the lower extremities such as 
plantar fasciitis and Osgood-Shlatters.
4) Instruct participants on the use of the FDM to treat conditions in the head region including 
headaches, TMJ, and hearing issues.

Course Fees
Cost of Course includes lunch and the books: Introduction to the FDM and Why Does It Hurt? 
AFDMA Member  - $700
Residents - $450
Active Military - $600
Medical Students (DO/MD) - $100
Nonmembers - $950  (or consider joining AFDMA when you register and save $50 today!) 

Registration will be limited to the following registrants:
15 physician or midlevel providers  
15 PT’s  
40 Medical Students (DO/MD)

Registrants must have completed either a Module 1 or Module 2 prior to registering for a Module 3. Please indicate when registering which Module 1 and/or 2 you have completed.

A waitlist will be available when each category has reached its limit.

Cancellation Policy 
Full Refund on or before 04/29/2016, 50% Refund 04/30/2016 - 05/13/2016.  No refund after 05/13/2016. However, course fee can be applied to future course. If course is cancelled by AFDMA at any point, a full refund of course registration will be offered.

Hotel Reservations
AFDMA has not established a room block at this hotel or any hotel in Yakima allowing registrants the opportunity to stay anywhere. It is highly recommended that a hotel reservation be made as soon as you register and hotel rooms are sometimes very hard to come by in Yakima and surrounding area. 

CME
The AFDMA has requested that the AOA Council on Continuing Medical Education approve this program for 16 credits of AOA Category 1A CME credits.  Approval is currently pending.

This Live activity, FDM Module 3 Seminar, from 05/22/2016 - 05/23/2016, has been reviewed and is acceptable for up to 16.00 Prescribed credit(s) by the American Academy of Family Physicians. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

All other healthcare professionals must submit their seminar certificate to their licensing organization to receive CEUs.
