Get Connected: VHQC Virtual Care Transitions Community and Resource Demo

VHQC’s Care Transitions Community offers a number of resources to help you improve care for the patients you serve. Join us on Tuesday, March 22 for a live demonstration of the community and the resources available to you and your community partners.

During the webinar, we will:

• Provide a live demonstration of VHQC’s Online Care Transitions community page with orientation about how to sign-on
• Demonstrate the features of each section with promotion of resources, including the Patient Health & Education Dashboard where the "widget" lives with patient ready diabetes tools
  
• Discuss the steps you can take to be an active member of the community such as posting items, asking questions and accessing resources
• Answer your questions about the community and the dashboard
