Region 3 DY5 Learning Collaborative - June 2016

Through the development and implementation of Region 3’s Delivery System Reform Incentive Payment (DSRIP) Program Projects, we have the opportunity to transform healthcare delivery in one of the country’s largest communities. This can be accomplished through thoughtful development of innovative and foundational projects as outlined in our Regional Healthcare Partnership (RHP) Plan. In order to ensure effective implementation of these projects and continuous quality improvement, We must review these projects and their impact throughout the life of the 1115 Medicaid Waiver.

Our Region’s second semi-annual event for DY5 will be held on June 8-9, 2016. At this event we will further our regional learning by:

Hearing about the future of the 1115 Waiver, through Waiver Renewal updates provided by HHSC
Learning about alternate payment models and their role in sustainability beyond the 1115 Waiver
Discussing issues around regional topics that impact providers throughout the region, including culturally competent care and quality improvement.
Gaining additional information on Cohort Workgroup activities, including opportunities to participate.
Reviewing regional and project progress and the impact to our local healthcare infrastructure, particularly through Category 3 Measures and Quantifiable Patient Impact Reporting (QPI) as we close out DY5.

At this event, a map of all DSRIP projects will be debuted and more information will be provided about Patient Navigation and Behavioral Health integration tools.

We look forward to an informational event and lively discussion with all stakeholders.
