CCCTTF - Hospital Readmission Reduction Summit 

Presented by the Contra Costa Care Transitions Task Force

Who we are: The Contra Costa Care Transitions Task Force is a group of health care and social service providers improving care transitions and reducing avoidable hospital readmissions.

Mission: To bring cohesion across the entire care continuum to ensure a high standard of care and patient safety.

We accomplish this goal by partnering with hospitals and community providers on care transition efforts to support county-wide process improvement.

Opening Session: Partnership for Readmission Reduction Summit
Objectives for the Summit

  • Identify key issues affecting successful care coordination in Contra Costa County.
  • Articulate how the county is addressing readmissions and medication safety.
  • Learn about existing improvement efforts among transitional care settings.
  • Illustrate how alternative payment models aim to improve transitional care across the continuum.

Session 1:  CRE Prevention, 4th Generation Antibiotics and Effects on Readmission
Learning Objectives

  • Understand the scope and significance of carbapenem resistant Enterobacteriaecae (CRE) in SF Bay Area
  • Describe the factors that contribute to CRE transmission and CRE incidence in Contra Costa County (CCC)
  • Understand the basis for infection prevention and antimicrobial stewardship interventions to reduce CRE
  • Identify opportunities for CRE Prevention across the continuum of care in CCC 

Session 2: County Readmission Rates and Regional Care Coordination Partnerships

  • Identify key issues affecting successful care coordination in Contra Costa County.
  • Articulate how the county and other regional partnerships are addressing readmissions and medication safety.
  • Learn about existing improvement efforts among transitional care settings.

Session 3: Unrepresented Adults in the Hospital and Effect on Readmissions

  • List the three identifying characteristics of an “unrepresented patient.”
  • Understand the elements of informed consent and how they are legally fulfilled for unrepresented patients.
  • Person centered care strategies to empower front-line and administrative staff to better work with unrepresented adults.

Session 4: Health System Transformation: CMS Priorities and the Post-acute Care Population
Learning Objectives
Review CMS priorities for acute and post-acute care, including:

  • Discuss trends and current quality standards
  • Review status of current CMS initiatives related to hospital readmissions reductions and QI in acute and post-acute care settings
  • Review new legislation/regulations related to acute and post-acute care of Medicare and Medicaid beneficiaries

Session 5: Bundled Payments and Effect on Readmissions

  • Comprehension of a new Medicare payment model
